Purpose of Forms: International students can use forms to submit updates, report required information, or request authorizations from our office such as an approved reduction in enrollment, or type of work authorization. All forms are linked below. Most forms are electronic forms that students can access in their IPConnect portal. Some requests may require the additional feedback of an academic department to confirm academic details.
Processing time: Requests can take about 2 weeks to process, while we balance fairness, efficiency, and sensitivity to government reporting timelines.

International students must maintain full-time enrollment unless approved for one of the specific exemptions listed below.

- Final Term Reduced Course Load: Use this e-form if you’re an undergraduate student, graduate student, or a student participating in an INTO OSU program who believes they are eligible to enroll less than full-time during the final term(s) of their program of study.
- Academic Difficulties: An Academic Difficulties Reduced Course Load is available to degree-seeking students who are experiencing academic difficulties related to one of these reasons:
- Initial difficulties with English language requirements
- Initial difficulties with English reading requirements
- Unfamiliarity with American teaching methods
- Improper course level placement
- Reduced Enrollment (Graduate Students): Reduced Enrollment is available for multiple terms to graduate students with a multi-term thesis, dissertation, or project requirement and have an up-to-date and signed Graduate Program of Study. A minimum of 3 graduate-level credits is required each term per the continuous enrollment policy for all graduate students.
- Masters of Science students with a project-based degree may be eligible for one or more terms of Reduced Enrollment to complete the project.
- Masters students with a thesis-based degree are eligible for up to 4 terms of Reduced Enrollment.
- Doctoral students are eligible for up to 6 terms of Reduced Enrollment.
- Concurrent Enrollment: International students who plan to enroll at Oregon State University (OSU) and another SEVIS-approved school at the same time should request permission for Concurrent Enrollment.
- Medical Reduced Course Load: To comply with USCIS regulations, a student’s medical condition must be substantiated by a licensed medical doctor (MD or DO) or licensed clinical psychologist before an International Student Advisor can approve a reduced course load. Students are allowed a total of 4 terms of approval per degree level.
- Vacation Term Request: Student visa holders may take a vacation once per each academic year. A vacation is a term during which you can be enrolled in either 0 credits or less than full time. Requires having 3 terms of previous consecutive full time enrollment, or be changing to a new degree program.
Sponsored Students
Students sponsored by entities such as a government or embassy can request documents from OIS to help them fulfill sponsorship obligations.
I-20 Requests
F-1 students will have a document called a Form I-20, “Certificate of Eligibility for Nonimmigrant Student Status.” Our request forms allow students to obtain updated I-20's, additional signatures, etc.

- Extend I-20: for when students learn they need more time to complete their degree than the I-20 program end date currently allows
- Replacement I-20
- Returning Student I-20: for when students does not have an active SEVIS record and would like to return to OSU
- Travel Signature Request: one of the items needed for international travel upon re-entry to the U.S.
DS-2019 Requests
J-1 students will have a DS-2019 document and can place requests to OIS related to this.
SEVIS Transfers
- Transfer-In F-1: E-Form will be provided to transfer students upon full admission
- Transfer-In J-1
- Transfer-Out F-1/J-1: For when students are fully admitted to another educational institution within the U.S.

Form I-765
Form I-765 is not an OIS form, but a US Citizenship and Immigration Services (USCIS) form, for filing for Optional Practical Training, or the STEM OPT Extension. An OPT or STEM OPT recommendation I-20 from OIS is required for filing with USCIS for OPT or STEM OPT otherwise an application to USCIS would be denied.
